He was predeceased by: his parents, Grace Herbert (Buckles) and Frank Costa, III. He is survived by: his significant other Virginia I. Sherman; his son Michael S. Herbert, Jr. of New Bedford; his brothers, John Herbert of New Bedford and Ronald Herbert of Maine; his daughters, Bethany Hess of New Bedford, Amanda Redanz of New Bedford and Kristi Botelho of Portsmouth, VA; and his grandchildren, Jaggar, Roriegh, Mason, Willow and Luke. He is also survived by several nieces and nephews.
