He is survived by: his parents, Dwayne "Butch" (Verla Mae) and Kuehn; his wife Shannon Mitchell; his children, Jennifer Sabaleski (Casey) of New Athens, IL and Jacob Hoffman (Lori) of Lenzburg, IL; his brothers, Kevin Hoffman of New Athens, IL and Marc Hoffman (Jennifer) of New Athens, IL; and his grandchildren, Levi, Ethan, Isaac Sabaleski, Annie and Clara Hoffman. He is also survived by other relatives and friends.
