He was predeceased by: his father Alex Kozuszek; and his brother Robert Kozuszek. He is survived by: his mother JoAnn Kozuszek of Nashville, IL; and his brothers, Jim Kozuszek (Janet) of Nashville and Kenny Kozuszek of Nashville. He is also survived by nieces, nephews, other relatives and friends.
