He was predeceased by: his father Gary Chouinard. He is survived by: his mother Linda Chouinard (Smothers) of Chandler, AZ; his fiancé Dawana Buchanan of Mt. Zion; his daughter Jilliana; and his sisters, Audrey of Aroma Park and Lesley of Kankakee.

Memorials may be made to Duane Dean Behavioral Health Center in Kankakee.
