He was predeceased by: his parents, Patrick James and Agnes Margaret Laverty O'Sullivan; his brother Bob O'Sullivan; and his sister-in-law Donna O'Sullivan. He is survived by: his wife Ann O'Sullivan of Mt. Sterling; his daughter Colleen Loehr (Mat) of Versailles; his granddaughters, Melanie and Jamie; his brother Jerry O'Sullivan of Frankfort; and his sister-in-law Judy O'Sullivan of Orland Park. He is also survived by many cousins, nieces and nephews.
In lieu of flowers memorials are suggested to Holy Family Catholic Church, Illinois Nurses Foundation, or Blessing-Rieman College of Nursing and Health Sciences Scholarship Fund.
