He is survived by: his parents, Jim Fortman and Dianne Fortman (Fisher) of Peoria; his children, Brandon Fortman of Morton and Brooke Fortman of East Peoria; his sister Jill Perkins (Barney) of St. Augustine, Fla.; his nephew Joshua Perkins (Kristin); his niece Nicole Perkins (Liz); his aunts, Jackie Fortman and Jane Fisher; his cousin Julie Ciota (Nathan); his nephew Dan Shaw; and his girlfriend Shannon Auperle Gaunt.
