With profound sadness, we say goodbye to Jerome J. Kozie D. D. S. of Morton Grove, Illinois, born in Chicago, Illinois, whose vibrant spirit touched the lives of many. He was predeceased by: his father Leon P.; his stepdaughter Amy Joy Olson (Vickie Ungar); and his siblings, Edwin Kay (Julie), Daniel (Helen), D. S. and Lee J. Kozie, Sr. (Shirley).
He is survived by: his wife Diane Sundblom; his son Jay Kozie (Petrina); his grandchildren, Thomas and Ellen Kozie; his stepgrandchildren, Kristin Mazzacano (Dan), Lauren Essert (Eric) and Emily Olson; his great grandchildren, Kai, Mackenzie and Abigail; and his godson Ted Kuczek.
