She is survived by: her children, Nancy Graham (Rob) of Warrensburg, MO, Nola Leslein of Dubuque, IA, Keith Forth (Pat) of Antioch, IL and Kent Forth (Kathy) of Morrison, IL; her grandchildren, Shea Leslein (Meghan), Laura Ward (James), Tara Thuriot (Paul), Audrey Forth, Anthony Forth (Maggi), Brian Forth, Kevin Forth (Katie), Kelly Forth and Kurt Forth (Erin); and her great grandchildren, Thomas, Wil, Sophie, Emmy Leslein, Kaylee, Alexa Ward, Collin Thuriot, Brooklyn, Addison Forth, Kendall, Kale Forth, Aiden and Andrew Forth.
