He was predeceased by: his parents, Benito Ortiz and Juana Ortiz; his wife Gay; his siblings, Mary Perez, Richard (Dick), Angel, Tony (Optician), Dan, Jose, Juan and David; his sisters-in-law, Margaret, Ruby and Patricia; his nephews, Rick Perez and Scott Ortiz; and his niece Lisa Marie.
He is survived by: his grandchildren, Laurie (Geno), Woolsey of Mahomet, IL, Taylor (Sean), Hershman of Downers Grove, IL, Ben Ortiz of Denver, CO and Georgia Legg; his great grandchildren, Addisen, Miles, Erin Woolsey, Quinn and Charlie Hershman; his children, Dawn Ortiz-Legg (San Luis Obispo) and Dr. Tim (Karen); his siblings-in-law, Ray Perez and Judy Ortiz; and his close friend Rita DeAngelo. He is also survived by nieces & nephews.
Phil's contribution to humanity was often recognized, including the Paul Harris Fellowship, Lions Club International, Morris IL Citizen of the Year, to name just a few.
