He was predeceased by: his father Earl Perkins. He is survived by: his mother Phyllis Perkins (Alexander) of Monticello; and his siblings, Jerry Perkins (Lau Rae) of Monticello and Cathy Warmbier of Monticello. He is also survived by several nieces, nephews, great nieces, and great nephews.
