He was predeceased by: his father Harold E.'Gene'; and his brother Rick Hoffman. He is survived by: his mother Norma Jean Hoffman (Cook) of Monticello; his siblings, Ron Hoffman of Monticello and Debbie Rear (Jim) of Monticello; his sister-in-law Glenda Hoffman of Maroa; and his nieces and nephews, Daniel, Logan, Jessica (Josh), Anna Marie, Emma and Edyn.

Memorials may be made to the Piatt County Animal Shelter or the Monticello Athletic Boosters.
