He was predeceased by: his father Raymond R. Hayes; and his brother Michael Hayes. He is survived by: his mother Betty Hayes (Vinson) of Monticello; his wife Brenda Hayes of Monticello; his children, Brandon Hayes of Monticello and Katie Kirkum of DeLand; and his siblings, Sara Dubson (Chris) of Farmer City and Tim Hayes of Monticello. He is also survived by 4 grandchildren.
In lieu of flowers the family request memorials be made to the Patrick Hayes Memorial Fund in care of the State Bank of Bement in Monticello.
