He was predeceased by: his parents, Florian Kozak and Marguerite Kozak (Boullier). He is survived by: his wife Judy; his daughters, Janine Stalcup and Nicole Newell; his grandchildren, Kyle Andre', Alexander, Aaron and Madelyn; his sister Jeannette Germain; and his nephew Pascal Germain. He is also survived by many nieces, nephews and other relatives.
