He is survived by: his former wife Mary; his children, Cynthia Forrest (Michael) and Robert Zavada; his grandsons, Kevin Forrest and Daniel Forrest; and his siblings, Jack Zavada (Arlene) and Barbara Piercy (Rogers). He is also survived by children and grandchildren, and his cousins.
