He was predeceased by: his parents, Robert Lee and Mary E. Hornback (Johnston). He is survived by: his wife Jayme Hornback; his sons, Zachary Alan James, Ethan Paul Hornback and Chris Hornback; his brothers, Danny Hornback (Kris), Bobby Hornback and Mark Hornback (Rachelle); and his siblings-in-law, Mark Howard (Doreen) and Julie Sawyers (Jackie). He is also survived by several nieces and nephews.
