He was predeceased by: his parents, Albert Mizell and Virginia Mizell Fulkerson (Robertson); his stepfather John Fulkerson; his wife Imogene Mizell; and his sisters, Patricia Collier, Wanda Pollard and Alice Pollard.

He is survived by: his children, Cindy Mizell, Carmen Beeding, Jack Mizell II (Tammy) and Sam Mizell; his grandchildren, Joel Kennedy, Lasha Kommer, Emily LaMarre, Benjamen Kennedy, Grace Kennedy, Rose Kennedy, Jake Mizell, Kristi Gillette, Drew Mizell, Cheyenne Mizell and Cierra Mizell; and his siblings, Mary Duncan and David Mizell (Marie). He is also survived by several nieces and nephews.
