He was predeceased by: his parents, John Anthony Perrotta III and Helen Louise Perrotta; and his brother John Perrotta (Ruth). He is survived by: his wife Barbara Ann Perrotta; his children, Dennis Neophitos (Cynthia), Christopher Neophitos (Danielle Coughlin) and Adrianna Neophitos; his siblings, June Proietti (Angelo), Paul Keating (Kathy) and Albert Keating (Rose); his grandchildren, Alexis Phylactos, Stephanie Phylactos, Demetra Phylactos, Nikoleta Phylactos, Carlos Garcia, Connor Coughlin and Paisley Neophitos; and his great grandchildren, Aubrey Phylactos, Emma Flores, Alana Flores and Willow Gates. He is also survived by cousins, nieces, nephews, great-nieces, and great-nephews.
