He was predeceased by: his parents, Thomas Petak and Thelma Petak (Mironov). He is survived by: his wife Marguerite Petak of Mattoon, IL; his children, Marlene Petak, LeeAnn Petak and Michael Petak; and his granddaughters, Emily Petak (Zachary Haben), Allison Petak and Anyssa Petak.
Visitation was held on Tuesday, March 25th 2025 from 11:00 AM to 12:00 PM at the Schilling Funeral Home (1301 Charleston Ave, Mattoon, IL 61938). A funeral service was held on Tuesday, March 25th 2025 at 12:00 PM at the same location.
In lieu of flowers, memorial donations in his honor may be given to the Lewy Body Dementia Association (LBDA.org), Moore Animal Rescue (http://www.mooresrescueranch.org), or a charity of the donor's choice.
