He was predeceased by: his parents, Frederick H. Lewis, and Ellen P. Lewis, Sr. (Jenson); and his grandparents, George, Harriett Lewis (Howe), Charles and Nina Jenson (Davis).
He is survived by: his wife Marilyn Knop; his daughter Rebecca Sloan (Louis); his grandsons, Luke, Seth, Daniel and Jesse Sloan; his son C. Matthew Lewis (Kara Greene); his grandson Chase Lewis; his son Michael D. Lewis (Carla Oliveira); his granddaughter Penelope Lewis; his brother George Charles Lewis (Judy Holmes); his aunt Mary Ann Plakovic; and his sisters-in-law, Sharon Kulhman (Bill), Patty Oliphant, Susan Malone and Debra Richmond. He is also survived by many other friends and family including nieces and nephews.
In lieu of flowers, memorials may be made to Maple Creek Hospice of Sparta, IL, American Cancer Society Hope Lodge in St. Louis, MO, or St. Jude Children's Research Hospital.
