He was predeceased by: his parents, Paul Jacobsen and Norma Jacobsen; and his brother Peter. He is survived by: his sisters, Chris Eigenhauser (Larry) and Joie Jacobsen; his nieces, Kelly Eigenhauser and Hanna Jacobsen; his nephew Brian Eigenhauser; his great-nephew Hudson Eigenhauser; and his aunts, Ellen Stuffle (Jack) and Joyce Cosman.

In lieu of flowers, contributions may be made to Shriners Children Hospital.
