He was predeceased by: his father Keith Rolland Cook, Sr. (Keith). He is survived by: his mother Mary Cook; his wife Sheleena Long; his children, Rebecca, Elizabeth Dewitt, Katlynn, Breanna Long and Gracie Cook; and his brother Jason Cook. He is also survived by other relatives and friends.
