He was predeceased by: his father Van Zandt. He is survived by: his mother Michelle Hook (Primeaux); his wife Laura Grass; his pet Kota; his sister Vanessa Hook; his nephew Ian Pusz; his parents-in-law, Linda and David Grass; his sister-in-law Sarah Sallinger (Eric); his niece Emma; his nephew John; his sister-in-law Erin Evans (Clint); his nieces and nephews, Grace, Claire and Wyatt; his brother-in-law Andrew Grass (Brooke); and his niece Isla. He is also survived by aunts, uncles, cousins and friends.

In lieu of flowers, memorials may be made to the family for a fund to be established at a later date.
