He was loved and cherished by many people including: his parents, William J. Cook and Dorothy J. Cook (Hettesheimer); his wife Shelly J. Belcher; his daughter Stacey Schultz (Jim); his grandsons, Jackson and Samuel Schultz; his mother-in-law Rosa; and also Shelby.
In lieu of flowers, a memorial will be established.
