She was predeceased by: her parents, Daniel Kotek and Catherine Kotek (Michuda). She is survived by: her close friend Brian Thome; her stepson Nick (Mary); her granddaughter Kendall; her siblings, Susan, Mary and Jimmy (Sarah); her nieces and nephews, Danny (Danielle), Steven, Jeffrey (Mikayla), Jonathan, Kyle, Timmy, Grayson, Hannah, Gracie, Liz, Amanda, Kelsey, Alicia, Carrie, Adrienne, Natalie and Kimberly; her father-in-law Raymond; her siblings-in-law, Julie (Chris), Terri (Greg), Tim (Barb), Mark (Sandy), Jeff and Bill (Dianne); and her pet Sadie.
In lieu of flowers, memorial donations may be made to the National Multiple Sclerosis Society, or to the family to be used as a college fund for Theresa's granddaughter.
