She was predeceased by: her parents, Guy Walter and Mary Ann Martin; her husband Danny Polen; her stepson Matt Polen; and her siblings, George and Ruth.
She is survived by: her son Corry Martin Rusty (Joanie); her grandchildren, Ashlyn, Taylor, Katlyn, Austin and Nathan; her boyfriend David Hackett; her partner's children, Jason Hackett (Crystal) and Dani Hackett; her stepchildren, DJ Polen and Mandy Walters; her grandchildren, Colton, Thomas, Keaton, Hayden and Dylan; and also, Devin, Sophia and Catalina.
Memorial donations may be made to the Susan G. Komen Breast Cancer Foundation and will be accepted at the funeral home.
