He was predeceased by: his parents, Chester Cook and Frances Cook (Walker); his stepson Richard W. Carlyle; and his stepgrandchildren, William Carlyle and Toni Carlyle.
He is survived by: his wife Sandy Cook; his stepson Victor Robert Carlyle III (Laurie); his stepgrandsons, Christopher Robert Carlyle, Michael Richard Carlyle and Jeffrey Jay Carlyle; his stepgreat-grandchildren, Joshua Carlyle, Victor Carlyle, Abby Carlyle and Robert Carlyle; and his sister Judy Harms. He is also survived by nieces, nephews, church family and friends.
