Mayworm (Libertyville, Illinois), born in Chicago, Illinois, who passed away on April 2, 2020, at the age of 63, leaving to mourn family and friends. He is survived by: his wife Suzan; his children, Brendan, Ashley and Anna Mayworm; his parents, Daniel Mayworm and Shirley Mayworm; and his siblings, Jeff Mayworm (Kay), Tim Mayworm, Patrick Mayworm (ChiMai), Christine Mayworm and Mary Mateer (Jeff). He is also survived by 9 nieces and nephews; and many cousins and dear friends.
