He was predeceased by: his wife Beatrice Slawson; his children, Louis Madalon and Mary K "Babe" Maggio (Mike); his parents, Roscoe Slawson and Gertrude Slawson; and his siblings, Bud Slawson and Phyllis Chandler.
He is survived by: his daughter Jackie Starzak (Michael); his grandchildren, Kristy Nondorf (late David), Kitty Potts (Randy), Karla Macek, Michael Maggio, Larry Madalon, LuAnn Radziejeski (Mike), Linda Copeland (John) and Melissa "Missy" Wilson; and his daughter-in-law Diane Madalon.
In lieu of flowers memorial donations may be made in Jack's name to Trinity Lutheran Church, Lansing, Illinois.
