He is survived by: his mother Wieslawa Bazan (Ledzwa); his wife Agnieszka Bazan (Zawadzka); his children, Daniel Bazan (Lauren) and Pamela Bazan (Mackenzie Doane); his niece Natalia Bazan; and his siblings-in-law, Beata Zmarlak and Marek Zawadzki.
