She was predeceased by: her former husband Tom Slymon; her siblings, Luis Gilberto Perelez (Tito), Edna Perelez and Virginia Marerro; and her parents, Swithem Perelez and Isabel Aponte.
She is survived by: her children, Amira Ballin (Shawn) and Soraya Faber (Eric); her grandchildren, Robert, Maria Ballin, Thomas, Liam and Sam Faber; her sisters, Doris Perelez and Evelyn Perelez; and her spouse Irishman (Tom Slymon).
In lieu of flowers, memorial donations in Nelly's name to Hesed House (hesedhouse.org) are appreciated.
