He was predeceased by: his parents, Donald and Frances Cook (Moore); his wife Linda; and his sister Judy Bruch. He is survived by: his children, Pam Wiegartz Leveroni (Tom) of Wrentham, Mass., Lori Haussmann (Cook) of Monroe Center and Jason Cook of Woodstock; his grandchildren, Brody, Jack, Reagan, Delaney, Dominic, Owen and Kingston; his family Hank Williams Jr.; his close friend Penny Verkuilen; and his siblings, Darlene Bruder of Kirkland, Diane Lee of Mendota, Vicki Brown of Monroe Center and Don Cook of Rockford. He is also survived by numerous nieces and nephews.
In lieu of flowers, memorials can be made in his name to Kirkland Fire Department.
