He was predeceased by: his parent Dale Miles Clark Sr.. He is survived by: his mother Shirley Sumbles; his sister Pamela; his brother-in-law Jeffrey Clark of Lake Villa; his niece Taylor; his nephew Robert; and his former spouse Robin Miller.

Memorials may be made to OSF Hospice Care, or to the Kewanee Humane Society Animal Shelter.
