She was predeceased by: her husband Jesus Lavin; her child Ric Lavin; and her parents, Ann Sakowicz and Bernard Sakowicz. She is survived by: her children, Rebecca Lavin, Raquel Woodward (Jack), Phil Lavin (Jen) and Reneė Villanueva (Ricardo); her grandchildren, Justin, Stacie, Beth (Kevin), Jason, Maggie (Don), Cody (Shannon), Luke, Natalie and Jake; her great grandchildren, Aiden Emily, Olivia, Nora, Max, Owen and Molly; and her sister Jill (Brian). She is also survived by nieces, nephews, relatives and friends.

In memory of Phyllis, donations to the St. Jude Children's Research Hospital are appreciated.
