She was predeceased by: her spouse Edward Ptak; her child Roman Ptak (Danuta); her parents, Anna Krzak and Jakub Krzak; and her sibling Tadeusz Krzak (late Jadwiga).
She is survived by: her children, Janusz Ptak (Boguslawa) and Malgorzata Ptak (Zbigniew); her grandchildren, Katarzyna Rzepska (Arek), Piotr Ptak (Kinga), Krzysztof Ptak (Magdalena), Agnieszka Green (Michael), Natalia Karcz (Rafal), Adam Kuta, Jakub Ptak and Pawel Ptak; her great grandchildren, Wiktor Rzepski, Elizka Rzepski, Klaudia Rzepski, Kasandra Rzepski, Dominik Ptak and Gabriella Green; and her siblings, Genowefa Mosson (Stanislaw) and Jan Krzak (Janina). She is also survived by many nieces, nephews, relatives and friends.
