He is survived by: his wife Phyllis Velazquez; his children, Rene Velazquez (Josie), Cynthia Velazquez (Armando), Yvette Velazquez (Steve) and Nikki Velazquez (Gabriel); his grandchildren, Mark Barco (Diana), Joseph Sanchez, Jessica Velazquez (Sal), Michael Velazquez (Melissa), Victor Velazquez, Jacob Cambray and Vanessa Velazquez; his great grandchildren, Yolanda, Lexi, Mia, Delilah, Abel and Josiah; his siblings, Jackie Coronado (Hector), Nadine Cruz (Raul), Donna Velazquez, Cherryl Brann and Jay Velazquez (Betsy); and his mother-in-law Rose Mayorga. He is also survived by nieces, nephews, relatives and friends.
