He was predeceased by: his siblings, Stefania Klimek, Janina Grela (late Bronislaw) and Ludwik Kwak (Janina). He is survived by: his wife Teresa Kwak; his children, Krys Shreffler (Scott) and Lisa Cheney (Richard); his grandchildren, Lauryn Shreffler, Colten Hines (Olivia) and Wesley Hines; his sibling Jozef Kwak (Lucyna); and his goddaughters, Teresa Florek, Barbara Kopinski and Irene Foley. He is also survived by numerous nieces, nephews and relatives.
With respect to State of Illinois Guidelines of Phase 4 during this time of Covid -19, the visitation is limited to 50 people or less at any time, we ask those attending to please keep one's visit brief to allow all to attend.
