He was predeceased by: his father Harold Butler. He is survived by: his sibling Orchards; his wife Kathleen Bagley Formidoni; his stepson Cyrus; his mother Ellen Butler; his siblings, Ellen Butler (Len), John Butler (Kathy), William Butler (Pamela), Harry Butler (Jane), Kevin Butler (Kathy) and Patricia Reyes (Jacinto); and his nieces and nephews, Grace, Ashley, Holley, Emiley, Angeley, Josephine, Isabelle, Maeve, Brendan, Jack, Julian, Ian and Matthew.

“So stunned to hear of Martin’s passing. I was a teammate of his in Little League as well as Mundelein High School baseball. We recently re-connected in May at an MHS baseball game and then again for a get together with other former MHS baseball players. The photo below is when we were 15 years old. I am next to Martin in the photo.”
