He was predeceased by: his parents, Frank Kozik and Judith Kozik. He is survived by: his sisters, Diane Grafer (Scott) and Lisa DeVaux (Joe); his nieces, Katherine Crowe (Connor) and Meghan Yocum (Sean); and his great-nephew Joshua.
