He was predeceased by: his parents, Charles Wilford and Anna Barbara Kalman Slack; his spouse Jarie Ann Stephens; his sisters, Barbara McIntire, Marjorie Christie and Donna Baseley; and his granddaughters, Jeanine and Jennifer Dassinger.

He is survived by: his children, Brian Slack (Kim) of Reno, NV, Robin Dassinger of Chicago, IL and Kelly Jacobsen of St. Charles, IL; his stepchildren, Amy Nelson-Stafiej (Ron James) of Island Lake, IL, Lori Eshoo (Rob) of Glenview, IL, James Nelson of Hopedale, IL, Eric Nelson (Nicole) of Island Lake, IL, Cindy Brasseur of Kirkwood, MO, Darrin Peterson and Larry Peterson of Jonesborough, TN; his grandchildren, Sabrina, Diane, Carl, Allison, Alexander Slack, Jason Dassinger, Erin, Zachary, Joshua, Noah Jacobsen, Brittany Stafiej, Michael Stafiej (Erin), Hannah, Kaylee Eshoo, Jimmy Jr., TeLisa, Payton, Taylor Nelson, Lucas and Georgia Nelson; and his great grandchildren, Addison and Evan Stafiej. He is also survived by many nieces and nephews.
