She is survived by: her husband Ray Slack; her children, Amy Nelson-Stafiej (Ron James) of Island Lake, IL, Lori Eshoo (Rob Eshoo) of Glenview, IL, James Nelson of Oklahoma, OK and Eric Nelson (Nicole Nelson) of Island Lake, IL; her stepchildren, Brian Slack (Kim Slack) of Reno, NV, Robin Dassinger of Chicago, IL and Kelly Jacobsen of St. Charles, IL; her grandchildren, Brittany Nicole Stafiej, Michael J. Stafiej (Erin Stafiej), Hannah, Kaylee Eshoo, Jimmy Nelson Jr., Telisa, Payton, Taylor, Lucas, Georgia Nelson, Sabrina Slack, Jason Dassinger, Erin, Zachary, Joshua and Noah Jacobsen; her great grandchildren, Addison and Evan Stafiej; and her brother Mike Stephens of Morton, IL.
