He was predeceased by: his wife Donna Cook; his parents, Clement Cook and Elizabeth Cook; his siblings, James Cook (Patricia) and Nancy Carson (late Alan); and his siblings-in-law, James McNealy (late Lorraine) and Joanne Kampton (Nikolas).

He is survived by: his children, Cathy Carroll (Al), Christine Cook, Kevin Cook (Cathy), Charles Cook and Colleen Cook; his grandchildren, Robert (Morgan), Sean (Molly), Colin Carroll (Caroline), Quinn, Brendan, Mary Margaret Creamer, Curran, Ian and Maeve Cook; his great grandchildren, Julian and Ruby Carroll; and his brother-in-law Kenneth McNealy (Margaret).
