He was predeceased by: his parents, Charles Kwiatkowski and Rose Kwiatkowski; and his siblings, Charles Kwiatkowski (Irene), Elaine Lipa (late Pete) and Alice Labak (Stanley).
He is survived by: his spouse Carolann Kwiat; his children, Kevin Kwiat (Jennifer), Sharell Sandvoss (Ken) and Kimberly Rieck (Marc); his grandchildren, Jonathan, Julia, Tyler, Ryan, Alexis, Elaina and Gareth; his brother Ralph Kwiatkowski (late Phyllis); and his close friends, Bob Bitunjac, Rich Vish and Dan.
Even as the world faced the COVID pandemic, Dan relentlessly sewed over 700 masks and distributed them to local healthcare centers and everyone he met.
