She was predeceased by: her parents, Ollie W. Reynolds and Pearl Irene Reynolds (Bell); and her brother Edwin Reynolds. She is survived by: her husband Herman Bontjes of Havana; her children, David Bontjes (Lauri) of Davie and Suzette Grant of Canton, Illinois; her grandchildren, Jacqueline Bontjes of Charlottesville, Virginia, Martin Bontjes of Los Angeles and Marissa Grant of Dumfernline, Illinois; and her stepgrandchildren, Michaela Halvorson and Zach Grant.
Memorial donations may be made in Hazel's name to the Alzheimer's Association.
