He was predeceased by: his parents, Albert Warren and Bonita Pearl Poole Mayhall; his brother Kenneth W. Mayhall; and his friends, John T. and Christie Brown.
He is survived by: his brothers, Leonard (Pam Mayhall) of Sikeston and Albert Mayhall of Washington, Indiana.; and his nieces and nephews, Nicholas Mayhall, Jessica Torres, Bonnie Haaff, April Mayhall, Amy Mayhall, Kenny Mayhall, Bobbi Holland and Timothy Mayhall. He is also survived by Several aunts, uncles and cousins.
