He is survived by: his children, Shellie, Michael (Samantha) and Eddie Burlack (Hollie); his grandchildren, Justin, Jeremy, Madison, Mason and Peyton; and his siblings, Flora, Carolyn (Charles) and Michael Burlack (Susan).
