She is survived by: her husband James E. Polak; her daughter Jolene Mira (Michael) of Boiling Springs, Pennsylvania; her grandchildren, Sienna, Auburn, Marianna and Alexandria; and her cousins, Diane Derby and Lorraine Tilley. She is also survived by other relatives and friends.
