She was predeceased by: her parents, Herbert Hoffman and Pauline Hoffman (Ogle). She is survived by: her children, Andrea Ax (Garrett) of Edwardsville, Ellen Ax Godfrey of Godfrey and Mark Ax (Sarah) of St. Louis, MO; her grandchildren, Emily Johnson and Brooke Godfrey; her brother Gary; her sister-in-law Penny Hoffman; and her nieces and nephews, Kelsie McCoy (Hoffman) and Kyle Hoffman of Jerseyville. She is also survived by other extended family and many friends.

Memorials may be made to the Illinois School for the Deaf, Jacksonville Community Center for the Deaf or the Cholangiocarcinoma Foundation.
