He was predeceased by: his parents, Leslie H. Cook and Helen Cook (Bray); and his daughter Cynthia Cook. He is survived by: his daughter Eugenia; his son-in-law Chris Wyatt of Godfrey; his grandchildren, Crystal Cook, Nathaniel Wyatt (Andi) and Ryan Wyatt (Kay); his great grandchildren, Alisa, Xavien, Alora, Annabelle, Colton, Kally Anne, Ainsley, Scarlett and Decklyn; and his siblings, John, Lynn Cook and Vivian Moon.

Memorials may be made to the 5A's Animal Shelter.
