He was predeceased by: his parents, Clyde A. Phelps and Melba Phelps (Nixon); his wife Claudette Needham; and his daughter Victoria Voght. He is survived by: his son Jerry Phelps Jr. of Godfrey; his grandchildren, Amanda Josefowicz and Clyde Phelps (Whitney); his great grandchildren, Pete and Paul Phelps; and his sisters, Martha Raby (Rayford) and Margie Graham (Dan).

Memorials may be made to a charity of one's choice.
