Smith of Freeport, IL.; her husband Mike Czernek of Glendale Heights; her sons, Michael Czernek, Jr. and Matthew Czernek; and her siblings, Chris, Matt and Georgia Grace Smith. She is also survived by many aunts, uncles, cousins and friends.
